How do I make an appointment?
New client contact may be made through email or by phone (click "Schedule a consultation" at the top of this screen). I will hold a brief phone consultation with all new clients prior to a first in-person/virtual consultation to gather some preliminary information, answer initial questions, and to begin to assess the goodness of fit between your early goals and my areas of expertise.

How long are appointments?
Initial consultations usually run a full hour. Regular psychotherapy sessions and play therapy sessions typically last 45-55 minutes.

Do you offer in-person or telehealth options?
I have a hybrid practice and offer both in-person and telehealth options.

Do you accept insurance?
Yes, I am currently an in-network provider for Aetna, Blue Cross and BlueShield, and Cigna/Evernorth. I am also able to help those who are interested in using a different insurance's out-of-network benefits. Clients should note that using their insurance does entail some risk and a puts a limit on confidentiality by engaging a third party to assess medical necessity and award benefits.
What is your payment policy?
I accept all major forms of payment including cash (exact change), credit card, electronic bank transfer (such as Zelle), or check made out to 'Lauren Formy-Duval, Psy.D.' For clients hoping to use flex spending accounts or health savings plans, I can provide a detailed statement demonstrating payment that you can submit to your insurance company for your reimbursement.

What are your fees?
If using insurance, rates will vary depending on plan copay, co-insurance, and any outstanding deductibles. For those paying out-of-pocket, the initial intake meeting lasts an hour and is $220. My fee for a 55-60 minute therapy session is $200. Many child therapy sessions are 45 minutes and have the rate of $175. I am able to adjust session fees on an individual basis depending on an individual/family's financial situation. *Fees listed are accurate as of 01/01/2024

What are your hours?
My practice is open on an appointment only basis. I do hold afternoon and some evening hours, though these are high demand and availability is limited.

Can you work with clients outside of North Carolina?
